Children benefit from an organized system of community resources that help them thrive – resources like health care, high-quality early learning experiences, healthy nutrition and parent support. And as a child’s brain develops rapidly during the first five years of life, there is incredible opportunity to build a strong foundation. But if the system isn’t organized well, it can be hard for families to access these resources and for service providers to connect families with the support they need. This can have long-lasting consequences on children’s health and well-being.
Help Me Grow (HMG) is not a stand-alone program. It’s a system that uses existing resources to develop a comprehensive approach to building early childhood systems in communities. The HMG model helps communities leverage existing resources in order to:
  • Promote the optimal development of all young children
  • Help identify concerns and needs among families with young children
  • Link families to community-based resources
  • Work in partnership with families by prioritizing parent and caregiver goals for their children’s well-being
  • Follow-up with families as well as their service and health care providers to ensure a cooperative approach to support

HMG is a framework that can be adapted to fit local context.

Across the country, Help Me Grow looks and feels different. But the existence and cooperation of the four Core Components define the model.

Centralized Access Point assists families and professionals in connecting children to the grid of community resources that help them thrive. 

Family & Community Outreach builds parent and provider understanding of healthy child development, supportive services available to families in the community, and how both are important to improving children’s outcomes.

When potential concerns are spotted early on, they can be easier to address. Child Health Care Provider Outreach supports early detection and intervention efforts and connects medical providers to the grid of community resources to best support families. 

To make sure the resource grid is working effectively, Data Collection & Analysis supports evaluation, helps identify systemic gaps, bolsters advocacy efforts, and guides quality improvement. 
It is the cooperation of these four Core Components that characterizes the Help Me Grow system model.
